RISE Clinic Nigeria is a prominent healthcare institution dedicated to improving community health in Anambra State, Nigeria. Established in 2012 through a partnership between RISE Global Health Initiative and the Godwin & Patricia Okeke Foundation, it aims to replicate successful healthcare models from developed countries within the local context.

The clinic’s mission is to enhance health and human development through interdisciplinary care and patient education. Its vision is to create sustainable health programs that increase access to quality healthcare, maximize resources, and reduce costs while promoting cultural diversity and tolerance.

Integrated Healthcare Delivery Model

RISE Clinic employs an Integrated Delivery System (IDS), coordinating various healthcare providers to ensure comprehensive patient care. This model supports community health workers, primary care providers, and specialists working together to optimize health outcomes. It aims to expand this model to other medical facilities in the region.
